Disabled Placards or Plates
What you will need to obtain a disabled placard:
- Application for Disabled Persons Special Parking Permit (TC96-204) (See Example)
- Section 1 must be completed by the applicant, signed and notarized.
- Section 3 must be completed and signed by a licensed physician.
- The applicant may request a second permanent placard.
- Permanent placards are valid for two (2) years from the date of issue.
- Temporary placards are valid for three (3) months.
Agencies or organizations that transport persons with disabilities as a part of their service may apply for disabled placards. The following are required:
- Application for Disabled Persons Special Parking Permit (TC96-204) (See Example)
- A statement from the director on agency letterhead verifying the need for the parking placards
- Name of the agency or organization requesting use of an accessible parking placard
- Number of vehicles being used in the transportation of persons with a disability
- The agency must submit vehicle identification numbers for all vehicles using disabled placards. These vehicles must be owned by the agency.

The placard(s) will be mailed to the owner within 3-4 business days.
What you will need to renew or replace a disabled placard:
- Completed Application for Disabled Parking Placard Renewal/Replacement/Additional (FCC0411) (See Example)
- Your application must be signed and your signature notarized.
Temporary permits cannot be renewed, you must complete a new application signed by a doctor.

What you will need to obtain a disabled plate:
- Application for Special Registration Plate for Disabled Persons (TC96-205) (See Example)
- Section 1 must be completed and signed by the applicant.
- Section 3 must be completed and signed by a licensed physician.
- The disabled applicant must be the registered owner of the vehicle.
- The original Kentucky Certificate of Registration
- Proof of current Kentucky liability insurance
- The Kentucky license plate currently issued to your vehicle must be surrendered.
Annual registration is $21.00
Disabled Decals
Any Kentucky resident who qualifies for a disabled license plate or permanent placard and currently has a "special plate" is eligible for a disabled decal. The disabled person must be the registered owner of the vehicle.
