Lost Boat Title

What you will need:

1. Affidavit for Replacement (TC96-167) (See Example)

Complete the following:

  • The upper half of the form with signature and notary
  • Check boxes to indicate the title has been lost

2. Application for Boat Title/Registration (TC96-184) (See Example)

Complete the following:

  • Boat Information Section
  • Owner/Buyer Section, including current owner's name, SSN and address
  • Check the appropriate boxes for the lien information
  • Signature and notary


$6.00 - Application notarized out of our office
$9.00 - Application notarized by the County Clerk's office

A registration will be mailed from our office. The new title will be mailed to the registered owner within 3-4 business days. (If pending a lien, the title will not print for 30 days.)

Acceptable Methods of Payment:
Cash, Certified Check, Cashier's Check, Money Order, MasterCard, Visa and Personal Check
A processing fee is charged on all credit and debit card transactions.


To request in person: Make an appointment to visit the Renewal Department

All appointments are at the Fayette County Clerk’s Office, 162 E. Main St., Lexington, KY 40507.

At your designated appointment time, proceed to the 2nd Floor entrance in the Helix Garage to check-in. (Do not use the front doors)  You will then be directed to the appropriate department for your appointment.

A mask will be required while inside the building.

Only the individual ording the duplicate title will be permitted in the building, anyone accompanying them will have to wait outside.

For all transactions processed by the renewal department please have all related paperwork in hand at the time of your appointment.  Do not make an appointment through this link for any transfers/change of ownership, or business related to the recording department (deeds, mortgages, marriage license).
